MEDP KN95 Masks and Face Shields Request Form

Yamhill Emergency Management has provided MEDP with 500 KN95 masks and 150 protective face shields for distribution to companies in McMinnville with less than 50 employees.  Please complete the request form below with your quantity preference, and we will do our best to accommodate your needs. Requests will be processed while supplies last.
